I am working on my first book. It is already some 70 pages. I have tried lots of things with mypublisher and find I am very frustrated in two areas. I may be missing something that more experienced know about.
I am frustrated that I cannot put titles on pictures and only have the text boxes.
I am really surprised that one cannot make titles different that than the text.
Am I missing something or is mypublisher really very limited in those areas?

You are correct.
One of the main issues involved is copyright and the license to embed fonts. Font foundries are very protective of their intellectual property (fonts) and will gladly sue other companies that infringe on copyright.
So, in order to include fonts in the BookMaker software when you download it, MyPublisher has to have the license to do so. And that ain't cheap! Likewise, to read the fonts that are installed on your computer and then embed them in the file you upload also requires license from the font foundry.
You do have options, though. Create a title page in Photoshop and use your own fonts. When you flatten the image and save as a JPG, Photoshop will automatically convert the text to curves (makes it an image) and you can use any font you want. Same for adding captions - either add captions on top of the image or extend the canvas size and add captions in the newly added white space.
