Ok, the cover page has 3 lines... what typically goes in here?

Hi Turbo-Dean -
It depends on what you want to say. Personally, I'd do:
Line 1: baby name
Line 2: date of birth
Line 3: place of birth.

There is no right or wrong way to do it though!
